What happened?
LightChat rig has a patrol agent (2e247cd5...) in a runaway loop spawning escalations. When triage requests reference missing beads, patrol creates escalations, which immediately spawn new triage requests referencing the now-deleted escalation, creating an infinite loop. The agent keeps creating new beads (dd1c2a6f, 49f6d467, c0582e18, etc.) every few seconds that reference deleted escalation beads, causing perpetual escalations about "bead not found" for escalations that were just deleted.
