Initial scaffolding for 4.0.0 release

We're moving from a 5 and 6 decimal release versioning (ex:
3.4.0.0.4and3.4.0.0.4.0signer) to a classicmajor.minor.revisionsyntax for both, starting with major release4.0.0. This is the initial scaffolding for that release.Checklist
